Online Project Status Report

Regular and timely reports are required from recipients of grants from the IEEE Foundation General Fund, the IEEE Life Member Fund, and the IEEE Humanitarian Technology Fund.  

Please read the guidelines for project reports available on this web site before completing this form.   The maximum number of characters (including any spaces) for each entry is specified and cannot be exceeded.   Complete all sections of this form and submit using the button at the bottom of this form.   If sections of this form denoted by * are not completed, you will not be able to submit your report.   The email address you enter in step 2, will be used by the IEEE Foundation to respond to you.

Step 1:   Identify your project

1.1 * Enter the "Project Code" provided by the IEEE Foundation

1.2 * Enter the project title you provided in your grant application form.


Step 2:   Identify yourself

2.2   If there are any changes to your project team, please provide details and an explanation for the changes.     (up to 300 characters)




Step 3:   Please provide the status report for your project in the following sections. Please refer to your grant application when you are completing the following sections.

3.11 * Explain what you have achieved so far with your project     (up to 1500 characters)

3.12 * Detail your progress with your project in terms of the steps and projected dates that you provided in your application. Please use percentage of progress in each step. You may have modified some dates - please use the actual ones and explain any changes.       (up to 1500 characters)

3.13 * Based on your experience to date, and if there are any changes from your answer to 3.13 in your application, describe how you will measure how the project has been successful. If there are no changes, simply state "no changes from the answer in my application".

3.14 * If your project will result in an ongoing activity after the project is completed, explain what is being done to ensure its continuity.

3.15 * Use this formatted Excel spread sheet to provide an updated financial statement for your project, showing the current status of all income and expenses, including all sources and amounts of funding. Please refer to the project budget that was approved by the IEEE Foundation.
Send your updated financial statement as an email attachment to
immediately following submission of this status report form.

NOTE: If this completed Excel spreadsheet is not sent to the IEEE Foundation as noted above, your status report will NOT be processed.


Step 4:   A copy for your records

You will automatically receive an email message containing a copy of your submission. However, since this form is designed in a printer-friendly format, you can use the PRINT button on your browser if you wish to make a printed copy of your completed form before you press the "Submit this form" button below.



Step 5:   Submit your Report By pressing the "Submit this form" button, I certify that all information I have provided is correct to the best of my knowledge. I understand that IEEE funding may be cancelled if this is not the case.
