Online Project Status Report for an IEEE Foundation Grant

Regular and timely reports are required from recipients of IEEE Foundation grants.   Please read the guidelines for project reports available on this web site before completing this form.   The maximum number of characters (including any spaces) for each entry is specified and cannot be exceeded.   Complete all sections of this form and submit using the button at the bottom of this form.   If sections of this form denoted by * are not completed, you will not be able to submit your report.   The email address you enter in step 2, will be used by the IEEE Foundation to respond to you.

Step 3:   Please provide the status report for your project in the following sections

3.1 * Project Background
Describe your project and state its goals and objectives in a brief project summary     (up to 1500 characters)

3.2 * Review the proposed milestone(s)       (up to 1500 characters)
List the key steps (milestones) for your project and for each one, describe exactly what has or has not been achieved. Where a milestone has not been completed, please state by when it will be accomplished.

3.3 * Use this formatted Excel spread sheet to provide an updated financial statement for your project, showing the current status of all income and expenses, including all sources and amounts of funding. Please refer to the project budget that was approved by the IEEE Foundation.

For your guidance, you may wish to view this sample interim Excel financial statement.

3.4 * Provide an updated cash flow for IEEE Foundation grant funds (up to 900 characters)
Show the month, year and amount for each payment for the duration of the project. For example, November 2004 - $250; February 2005 - $400.
