This case study will follow a specific incident from identification to final resolution. Detailed requirements, construction reports and photographs will be given to document the failure. A careful analysis of parameters prior to the failure will be described, as well as an on-site inspection of the vendor's production facility to ultimately locate the root cause. Final resolution of the problem will include procurement and qualification specification revision to ensure failure does not recur. The approach should provide broad insight into any engineering investigation, and hopefully into failure prevention.

Biographical Sketch

Mr. Kurt J. Clemente, P.E. is a Project Engineer/Program Administrator for Northrop Grumman Newport News. Since his employment with NGNN in 1997 Mr. Clemente has been involved in investigating design concepts and emerging requirements for shipboard electrical and telecommunications systems. Mr. Clemente has filed nine invention disclosures on various topics, and holds 3 patents in the areas of Fiber Optic Cable system capacity and shipboard lighting utilizing LEDs.

He is a registered Professional Engineer in the Commonwealth of Virginia and a Certified Commercial Electrical Inspector. Mr. Clemente is a member of BICSI and the IEEE. A member of IEEE, he has been a part the Hampton Roads EXCOM since 2005 and is Chair.

Mr. Clemente received a BS in Electrical Engineering in 1993 from the University of Illinois and a MS in Oceanography from Old Dominion University in 1995.
