Presentation:

Anatomy of an Accident: How Lack of Documentation Will Cause Problems

A pedestrian was electrocuted when she stepped on an electrified metal lid in the sidewalk. The subsequent investigation by the Texas Department of Transportation revealed that the designed grounding had been interrupted by improper maintenance. But, the question of whether grounding had actually been installed properly during installation was left unanswered because of the lack of documentation, which left the original contractor liable for damages. This presentation will explore the various activities, errors, and miss-steps which preceded the fatal accident.
