Required for Petition to Form an IEEE Chapter: (1) At least 12 IEEE members' signatures.(The members must be the Society's members)(2) The action has the approval of the Section Executive Committee.
1.Download Application Form:Chapter petition.doc
2.Read the explanation on the 1st&2nd page,and then fill in the "Chapter Petition" on the 3rd page. **12 Petitioners are required for a Technical Chapter Formation**
3.Mail or Fax to IEEE Tainan Section for Setion Officer's Signature.

4. Tainan Section will help email the completed chapter_petition form to IEEE. A couple weeks later, IEEE will inform chapter the formation of the new chpater via e-mail.
Note:The requirements of upgrading are (1)Significant performance in professional experience over a period of at least five years. (2) Three references from current IEEE members holding the grade of Fellow, Senior Member, or Honorary Member.
