Obtaining Visa Application Document (Formal Letter of Invitation):

The following form (*) is to obtain a supporting document for those attendees who need a visa to travel to Italy. This document is a formal Letter of Invitation recognized by the Italian government for the 2009 IEEE International Ultrasonics Symposium. Once you have filled out the form you have to submit it, a confirmation email will be sent to you (the email will be sent to you automatically after receiving your information). If the information submitted are complete, the formal Letter of Invitation will be sent to you via fax or via e-mail within 24/48 hours.

Therefore, please check with your travel agency, local Italian Embassies, or local Italian Consulates to find out what documents are needed for your visa applications.

Please note that the Letter of Invitation is no guarantee that you will get a visa but may be required by the Italian Embassy or Consulate as part of the attendee's visa application.

To ensure a timely delivery of the visa application document, the request for the document should be sent via the following form before the deadline, September 1, 2009.

(*) The letter of Invitation will be send to the applicant in two cases: 1) if the applicant have a paper accepted in the IUS program; 2) if the applicant has just payed the regular fee for the registration. In all other cases, it is not possible to send a letter of Invitation.
